Acknowledgment of Risk:

I, the undersigned player, and my parent/guardian, understand and acknowledge that participation in basketball and related activities with Rocktop Elite involves the risk of serious injury, including but not limited to broken bones, sprains, concussions, or even permanent disability. I voluntarily assume all risks associated with participation.

Medical Treatment:

I authorize Rocktop Elite staff, coaches, and representatives to seek medical treatment on behalf of the player in the event of injury or illness. I understand that I am responsible for all medical costs incurred as a result of participation.

Release of Liability:

In consideration of being allowed to participate in Rocktop Elite basketball programs, I, the player, and my parent/guardian, hereby release, waive, and hold harmless Rocktop Elite, its officers, directors, coaches, staff, and volunteers from any and all liability, claims, demands, or causes of action for any injury, loss, or damage, whether caused by negligence or otherwise, arising out of or relating to participation in the program.

I, the undersigned player and parent/guardian, grant Rocktop Elite, its representatives, and affiliates the right to take and use photographs, videos, or other media of the player during practices, games, events, or promotional activities. This includes, but is not limited to, use in print materials, social media, websites, or other marketing and promotional purposes.

I understand that the player will not receive compensation for the use of these images or videos. I release Rocktop Elite from any claims or liability arising from the use of such media.

I, the undersigned parent/guardian, give permission for my child, the player named below, to travel with Rocktop Elite coaches, staff, and team members to and from practices, games, tournaments, and related events.

I understand that travel may include transportation by team bus, private vehicles, or other arranged means, and I acknowledge the potential risks involved. I release Rocktop Elite, its coaches, staff, and volunteers from liability for any incidents occurring during travel, except in cases of gross negligence or intentional misconduct.

All players, parents, and guardians participating in Rocktop Elite programs are expected to uphold the highest standards of sportsmanship, respect, and integrity. By signing below, you agree to the following:

Respect Coaches, Officials, and Staff – Follow instructions, communicate respectfully, and accept decisions without argument.

Respect Teammates and Opponents – Treat all players with fairness, avoid bullying or harassment, and encourage a positive team environment.

Attendance and Commitment – Attend practices, games, and events on time. Notify coaches in advance of absences whenever possible.

Safety and Equipment – Use equipment properly, follow safety guidelines, and avoid actions that could harm yourself or others.

Behavior – No use of alcohol, tobacco, or illegal substances during team events or while representing Rocktop Elite. Refrain from inappropriate language or gestures.

Consequences – Violations of this Code of Conduct may result in warnings, suspension, or dismissal from the program at the discretion of Rocktop Elite staff.

Acknowledgment:

I have read and understand the Rocktop Elite Code of Conduct and agree to abide by its rules and expectations.

I have read, understood, and agree to abide by the Terms and Conditions as posted on RocktopElite.com at the time of completing this registration form. I understand that my participation in Rocktop Elite programs is contingent upon compliance with these terms.

I, the undersigned, confirm that I am over the age of 18 and am fully responsible for my participation in Rocktop Elite programs. I have read and understand all sections of this registration, including the Liability Waiver, Photo/Video Release, Travel Permission, and Code of Conduct. I agree to comply with all rules, regulations, and safety requirements.

I acknowledge that by signing this form, I am giving up certain legal rights and assume all risks associated with participation.
